    Does the spaces appear at the same places every time?

    Have you looked at the 'codes' (¶ in the Paragraph menu) at the blank spaces? There might be tabs that doesn't show in Word, but change when converting/exporting.

    I can put your Word file(s) (both original and altered) through my MS Word 2007 with Adobe plug-in and export them to PDF/X-1a:2001. Or you may have tried that yourself?

    No the spaces are not at the same places. I have gone through the whole document with show codes on. The only tabs are the ones I put there. I have CS 6 with inDesign and plan to re-learn it for my next book. But that doesn't help with this one which is 302 pages in word.

    You can 'Place' a docx into InDesign. There is a key combination to make InDesign expand the text to all pages when you click to Place.

    You can also copy/paste, but it works best with smaller portions of text, like chapters.

    I hope someone will show up with a solution to you.

    If you make a Shift-Click after selecting the docx, you get all the text inserted (depending on how many pages you have set up your InDesign document to hold.
